Goose Creek ISD Electronic Forms Direction Page

Welcome to the home page for the online forms required by UIL and Goose Creek ISD. These forms are required each year your son/daughter wishes to participate in athletics for GCCISD. The athletic department would like to thank you and your son/daughter for filling out the forms and submitting them to us in a timely manner.

To get started, you will need the student’s name and student number.

Directions:

1. Download and Print the Physical Form. The required annual athletic physical and health history cannot be filled out online because it must be completed by the parent/guardian and physician. You may print a physical and medical history by selecting the physical and medical history tab at the top of the page. Once you have selected the tab you will see the link to the physical and medical history and will be able to download and print.

2. Fill out all other required paperwork electronically. Parent/Guardian will be asked to provide information such as home phone numbers, cell phone numbers, email addresses, name of primary physician, insurance policy information and other pertinent information. Please make sure you have this with you when filling out your forms electronically.

3. Remember that to submit online appropriately, you and your child must fill out and sign in all required spaces. Forms will not be accepted electronically without all required information being completed.

4. To access the required athletic participation forms for Goose Greek CISD, hold your cursor over the “Electronic Participation Forms” tab. You will get a drop down list of all required forms. All forms in the “Electronic Participation Forms” tab must be completed.

5. To begin, click on the form name and fill out the information requested. Parent/Guardian must have their son/daughters present along with the students ID number available to complete the forms. If you do not know the answer to a question, please answer the question with N/A, or none.

6. To sign the document, click inside the signature box and hold your mouse down, this will allow you to create an “Electronic Signature.” If you make a mistake and need to start over, click on the refresh icon next to the signature box.

7. Once you have filled out all of the information on each page you will have the opportunity to print the document.

8. You will receive a confirmation email once the document(s) have been reviewed by GCCISD

El Departamento de Atletismo de Goose Creek CISD les da las gracias por tomarse el tiempo para completar los formularios requeridos de UIL y de GCCISD para participar en el programa de atletismo. Es requerido que todos los estudiantes atletas del 7mo al 12vo llenen y sometan estos formularios por internet antes de cada ciclo escolar.

Goose Creek CISD y el UIL requieren que el padre / tutor complete los siguientes formularios electrónicamente. Los estudiantes que van a participar en atletismo cada año escolar tienen que completar todos los formularios. Los padres y los estudiantes necesitan leer las formas de UIL que se relacionan con ellos. Todas las formas de este año se llenarán en línea para hacer el proceso más conveniente para todas las familias. La única forma requerida que no se puede hacer electrónicamente es el historial físico y médico anual. Los padres podrán imprimir las formas de historial físico y médico durante la sesión electrónica. El historial físico/ médico debe ser llenado por el padre y un médico. Se requiere un físico para cada nuevo año escolar. El historial físico y médico debe entregarse al entrenador de su hijo/hija o al entrenador de atletismo. El departamento de Atletismo de GCCISD le recomienda que haga una copia del físico para sus archivos.
